Skilled Nursing Center

Chosen Valley Care Center opened its doors to serve residents of the Chatfield area in March of 1976. The Center is licensed by the Minnesota Departmentt of Health to provide skilled nursing care to our residents. In addition, all beds have been certified to provide skilled care through both Medicaid and Medicare programs.

The special quality of Chosen Valley Care Center is reflected in the mission statement, our residents, our staff and our ongoing commitment to excellence. We provide a pleasing, home-like environment in which our residents receive nursing care and therapeutic rehabilitation.

Rooms

We want you to have the most comfortable stay as possible. Resident bedrooms offer a choice of private or shared living spaces. Rooms include comfortable/adjustable beds with bedding, over-bed lighting, nightstands, closet space with drawers and a bathroom. Connections to telephone, television, and wireless internet are also available. If you have specific questions about our rooms, or other amenities please inquire with our social services director.

Common Areas

Chosen Valley Care Center offers a wide-array of common areas for residents and families. Whether using the computer, watching a big screen television, reading a book, listening to the soothing sounds of a water fountain, or relaxing on one of our outdoor patios, there’s something for everyone to enjoy. We also offer space to reserve for small gatherings and special events. We do our very best to ensure our common living areas meet everyone’s needs.
